Polypharmacy

My journey here should be in the signature. I am NOT stable; have lot of anxiety, brain fog, sleep 5-6 hrs.

What is the plan? Go back to 3 mg Ativan and stabilize there? Switch to Valium afterwards?  Taper mirtazapine first? Add sertraline, as my doc wants?  Transform Ambien CR in regular or in 10 mg Valium?

I am lost. I need a plan. Can the community help, please?
