Liquid titration schedule

What percentage would you like to reduce by and how long do you want to hold each dose?  Do you know the strength of the liquid Lorazepam yet?

Were you planning to dilute the liquid ativan into water and remove some of the solution as you do each cut?

Do you have any preference on how much to dilute your lorazepam.  Some people use 300 ml; some use 100 ml.  Doesn't really matter.  Just personal preference.  We'll start with that volume and reduce it every three weeks.

 

Are you able to get the 1 mg/ml solution?

Assuming that you get the desired 1 mg/ml liquid formulation...

 

Get a 1 ml syringe and withdraw 0.625 ml of the liquid formulation.  Transfer that into a 100 ml container, add water to make a total of 100 ml, and mix well.  That will be your 0.625 mg initial dose.  You'll have to estimate a bit to get 0.625.  There should be a mark for 0.62 ml and a mark for 0.63 ml.  You could shoot for the middle, but I'd suggest you use the 0.63 mark.  Many people switching to a liquid formulation feel like they get a little less medicine, so the very slight increase might be helpful. 

 

When you're ready to make your first reduction, you'll make the solution as above then remove and safely discard 10 ml.  You'll drink the remaining 90 ml.

 

When you're ready to make your second reduction, you'll make the solution then remove and safely discard 19 ml and drink the remaining 81 ml.

 

For the third reduction you'll discard 27 ml and drink the remaining 73 ml.

 

4th reduction - Discard 34 ml; drink 66 ml

5th reduction - Discard 41 ml; drink 59 ml

6th reduction - Discard 47 ml; drink 53 ml

7th reduction - Discard 52 ml; drink 48 ml

 

That's several months worth of reductions.  I'm happy to do more math when you need it.  If you need adjustments made, I'm happy to try.  Send me a PM if you post something for me to a thread and I don't reply.

It's a MCT oil based.

 

Do I need to be mixing it in water?

I'm a little confused.

The Pharmacist, who is a specialist in de-prescribing, just said to take out the 2.5 ml the first week etc.

 

Just wondering if this is the same thing that you are talking about in this thread.

From what you posted you can certainly use that solution directly.  I imagine that people could also directly taper from a 1 mg/ml suspension, but to do it accurately might be challenging (especially at low doses).

I checked again and my liquid from the compounding pharmacy is a 0.1 mg/ml liquid.

 

So given this, I should be ok at the smaller doses correct?

Hi Winniedog, I'm new to this my pharmacy doesn't carry the liquid lorazepam so trying another pharmacy.  Hopefully someone will respond to your question who knows about this.  Sorry I couldn't help.

Thanks the liquid didn't work out for me.

I reacted to the MCT oil.

 

I have to stabilize.

 

I don't know how to do this.

 

My brain feels swelled everyday.

 

It's like I've been through close to a cold turkey, and still have to get off!!

 

I don't know how to do this!!!

 

I don't know if I need to switch over to something else to taper off.

The Ativan is too hard to get off for me.

I can only use the sublinguals, as I don't do well with the actual white pills.

 

I'm lost and alone......

MCT oil can take a little getting used to.  I think it's best to introduce it slowly into one's diet to give your digestive flora time to adjust.  Else, I think some people get cramping, runs, ...

The problem is that my entire body is screwed up from antibiotics in the spring.

I can no longer handle any additives and MCT oil, which is from coconut is a sallyicillate, which I can't handle.

 

I can only handle the sublinguals, how do you taper off those!

 

I'm really lost on how I'm even supposed to get off these F'ng pills.

 

I have so many symptoms, almost all of them, and I'm basically bedbound and non-functional.

 

I have no DR that will help, let alone believe me with what's going on.

 

I basically go to bed every night, wondering if I'll wake up.

 

this is terrifying!

While a sublingual taper is a bit unusual, I'm sure it can be done.

 

How do you currently take your pill?  Give a little detail.  I assume you suck on it until it's completely dissolved.  How long does it take to dissolve?  Is the pill coated?  Flavored?

I think that you can simply crush and weigh to do a taper.  Then put the powder under your tongue just as you currently do with a pill.  Easy Peasy.

 

All of the other symptoms sound to me like some combination of tolerance/dependence and anxiety.  Impossible to know how much of each.  Sounds like you're in the middle of a lot of 'stuff', and that you've got a lot going on in your head right now.  See what you can do to quiet those concerns down for a while.

Help I am so confused maybe I am overthinking  but I don't know!!!  My doctor is trying to help me with my taper but I really don't think she is very benzo taper savvy.  I asked for liquid lorazepam which she prescribed but upon reading the dosage that she prescribed and the pharmacy filled it doesn't make sense to me.  I am taking .625 mg/day of lorazepam (.125 mg am, .125 mg noon, .125 mg afternoon,  .25 mg pm).  The script for the liquid says .25 ml am, .25 ml noon, .25 ml afternoon,  .50 ml pm...this sounds like too much liquid???  Am I wrong???
