Colin - Here's the info you requested for my titration

Recommended Posts

A few months ago I cold turkeyed (not knowing any better) and it was a month of full blown withdrawal misery until I stabilized.

This month, my benzo prescribing doctor has been on vacation for two weeks, so I missed 4-5 days of my medicine and have gone into horrible withdrawals again, which I'm still trying to stabilize from.

In between those times, I've tried dry cutting, each on separate occasions, 5mg, 2mg, and even 1mg, yet by day 4-5 the withdrawals hit me really hard and I suffered horribly again for a few weeks, until my reinstatement kicked in again enough to calm my nervous system down.

Given that, I believe titration is going to be the best way for me to go. Can someone please help me out here and lead me in the right direction? I've read a lot here, but my benzo messed up brain just can't absorb all of it.

I'm currently taking 20mg Valium in the morning and 15 in the evening.  I have 2mg and 10mg pills.

I probably won't start decreasing my dosage for a month or so, or at the very least until I'm stable for a week.  I need a plan to take to my doctor ASAP though, before he causes me to drop my dosage way to fast at 5mg every two weeks, which he thinks is okay for me and it's not.

Given my sensitivity to withdrawing, at what rate should I consider? 2.5& or 5% every 2-4 weeks???

Many thanks to those of you who will reach out to me to help me in my desperate need.

Hi Christiana,

Your total daily dose of Valium is 35mg. A 1mg cut from that amount every two weeks equates to 3%, that is between your request of 2.5% to 5%. You can even cut your 2mg into 1/4s making .50mg cuts possible which would only be a 1.4% reduction.

THANK YOU VERY MUCH THERESA2!

I understand what you're saying.  Wouldn't titration allow me to even the percentage drop out though, so on day 4-5 I don't get hit so hard??? And that way if I need to hold at a few ml for a while, I can???

Most people titrate at 1% a day or 7% per week. With you just starting out cutting .50mg from 35mg, would equate to 1.4% every week or two weeks just a little more than most cut in one day. Cutting .50mg every two weeks will take you 140 weeks or almost 3 years?

THANKS AGAIN THERESA FOR TRYING TO HELP ME AGAIN!

I understand what you're saying.  I don't care how long it takes, as long as I'm moving forward.  My situation is complicated by the fact I took a fluoroquinolone antibiotic, called Levaquin, which is the reason why I was put on Valium in the first place.  I also have multiple chemical sensitivities, even to the point where any type of anesthesia or anesthetic makes me very ill.  On top of that, I also have chronic late stage disseminated Lyme disease, which has affected my central nervous system, in a big way.  I've also been diagnosed with fibromyalgia and chronic fatigue syndrome, but they are likely the result of me having Lyme and for so long.  I also have hypothyroidism and adrenal fatigue.  So, I have a host of chronic medical conditions that complicate my withdrawal process.

Given that, my central nervous system is EXTREMELY sensitive to any cut I make.  As has been said here, many times, everyone is different.  Flick is one that comes to mind who is having to withdrawal VERY slowly also, due to having a VERY delicate nervous system.

The problem is, I cut and get SEVERLY slammed at the 4-5 day mark and my body/brain doesn't have enough time to catch up, so I end up in SEVERE withdrawals to where I'm incapacitated and want to die.  I've also been on Valium for almost 3 years at a high dose, although, again, according to MY body chemistry, I don't know how much it's complicating my withdrawals.

I just feel like, for ME, if I can take a plan to my doctor, especially since he is FULLY aware of all of my chronic medical conditions, he will agree to let me do what is most comfortable for ME.

My thinking, at this time, is to start REALLY, REALLY slow, see how I do, and I can increase the process, based upon how I feel.  I just don't want to get into another situation where I'm so incapacitated again, I feel like I'm going to die.

I'd like to start with titrating 1mg over a month's period of time and see if that doesn't help the DRASTIC dip in my blood levels and SEVERE increase in my withdrawal symptoms I feel at my magic 4-5 day mark, when all the HORRIBLE withdrawal misery breaks loose and doesn't let up for weeks.

To complicate my situation, even further, I live alone, have very little support, have recently gone through a divorce, am having financial problems, thus may lose my home and end up homeless, because my ex husband is in contempt of court for not supporting me, as I'm disabled.  That means I have to go back to court soon, which is stressful itself.  So, my personal situation makes my life VERY stressful, on top of withdrawals.

Honestly, I DESPERATELY want off of the benzo and TRULY wish it was an easy process, but I have some VERY horrible stresses in my life, thus I have to keep my withdrawals to a VERY minimum.

I apologize if this is too much information, but I feel like I have to tell you all this, so you can see it's not fear that's causing me to feel like I need to titrate VERY slowly, it's my VERY stressful life circumstances, at the moment, as well as my chronic health problems, which have affected my central nervous system and made it EVEN more delicate.  Even so, I HAVE to start reducing my dosage, else I know I'm NEVER going to heal, so I'm ready, as long as I can keep my withdrawals to a minimum, but I need some help to titrate at a snail's pace.

Does all I've said put things into better perspective for you now? I hope so.  Please let me know.  I really need your help, which I appreciate from the VERY bottom of my heart.

All of the information is in my blog.  I don't know how to add the link to it here, but if you just type my name in the search box, you can find it.

I just don't know what to do.  Everyone says listen to their body about withdrawing and I am, yet I'm being told I want to go too slow.  Obviously, I'm scared OUT OF MY MIND, based upon how I've felt just trying to dry cut and SOOOOO confused, not to mention I don't want to and can't suffer SEVERELY, as I have NO ONE who understands enough to help me.  I CAN handle the mild stuff, but NOT the incapacitating stuff.  Right now, I DESPERATELY wish someone with A LOT of experience would have the heart to just reach out to me and contact me via phone.  It would be SOOOOO nice to hear someones voice on the other end of the phone and have their support with all this via more personal contact, since I have NO ONE in my life that understands withdrawals from benzos.  I think I'll just go have a really good cry now and come back later when I feel better.

Everyone is right: Listen to YOUR body.  Each situation is a unique one and yours seems to have alot of othere issues also.  Don't worry about how fast/slow as long as it is working for you.

I agree that titrating is gentler than dry cutting. It allows a mini daily cut rather than a huge cut once every 10 days or so.  Yes, the percentage may be the same, but it is done much more slowly giving the body time to adjust.

I dry cut down to .375 then titrated to stay level.  It worked for me :)

Hi Colin,

Here is the titration info you’ve requested.  Thank you, in advance, for your help.

Start Date – I’m still dealing with SSRI discontinuation syndrome, so I’m currently trying to heal from it, before I start titration.  I’m already so sick from it I don’t want to add benzo withdrawal on top of it, plus not be able to figure out which I’m suffering from, not to mention take a chance of getting even sicker.  I go back to see my doctor soon though, who is very supportive, and he’s has asked me to have a schedule in hand then, so I have a need to appease him, even if I don’t start right away.  For now, let’s just assume September 15th.

Dose – 35mg per day of Valium, where I take 20mg in the morning and 15mg in the evening.

Tablet Dose – 10mg; I also have 2mg, but I’m not using them, at this time, since I’m able to break one of my 10mg tablets in half to give me the 5mg I need to take everyday to total 35mg per day.

Number of Tablets – two 10mg in the morning and one 10mg plus ½ 10mg in the evening.

Taper Rate – I want to err on the side of caution and start with a rate of no more than 5% every 2 weeks and hold if I feel the need to or increase if I’m doing alright.

Cylinder Details – I plan to purchase a 100ml cylinder with 1ml increments.

Here is something else I need a schedule for.  Assuming I have a bottle of liquid Valium which is equal to 5mg per 1ml and want to taper at a rate of no more than 5% per every 2 weeks, how would I do that? What tools would I need to do that? I say this because I believe it will be less hassle and perhaps even more precise than titration, if I can do it that way.  I firmly believe my doctor will go along with it and as long as it’s cost effective for me to get the liquid, then I think this would be my preferred way to become benzo free.

So, for instance, before I start titrating, since I’m currently taking 35mg per day of Valium, on the first day, I’d take 30mg per day in pill form, then the other 5mg per day in liquid form.  That means starting out, I’d work on just getting rid of the 5mg and get down to 30mg.  Once I’m down to 30mg, then I’d take 25mg per day in pill form, then the other 5mg per day in liquid form and so on.  So my biggest question is, how to accomplish this with the liquid 5mg at a time, until I’m benzo free, without exceeding no more than a 5% rate of reduction of my full benzo dosage every 2 weeks???.

I’m sorry, but I just can’t seem to figure out all of the math.  On top of that, dry cutting is just way too hard on me.  I need a very gradual reduction in my daily dose, so that I don’t get slammed with horrible symptoms at what seems to be my magic 4-5 day mark, when it seems my body’s blood levels of benzo drop enough for me to continue to get worse for a week or so, until I feel somewhat better.

Again, thank you so much, in advance, for your hard work in creating these schedules for me.

THANK YOU VERY MUCH COLIN FOR LETTING ME KNOW!

Here's three titration schedules I've come up with on my own. PLEASE check my numbers and let me know what you think.  I'm well aware the rate of my titration may need to be changed, as I go along, especially as I near the end (I may even need to create a new schedule near the end), thus my reasoning for the three titration schedules I've created.  I think the 5% one is a good starting point for me though, just so I can play it on the safe side and get a handle on how I'm going to feel.  If I find I feel fairly well enough, then I know I can always increase my titration rate, by using one of the other titration schedules.

Here's what I need your help with the most.  Based on my titration schedules, can you tell me how I'd use liquid Valium of 2mg per 1ml using a 1ml syringe with 1/100ml increments, as well as milk titration with a 100ml cylinder marked in 1ml increments to accomplish reducing my dose on a daily basis? That's the part that confuses me the most! I think I'm getting there with the math, but I can't wrap my head around those two issues.  I just don't want to end up hurting myself really bad, by mistakenly causing myself incapacitating withdrawal symptoms, thus why I REALLY need your help to make sure I have everything right in my head, before I start reducing my Valium dosage.

If I can and were allowed by you, once you help me to get it all figured out, I'd be more than happy, with your permission and oversight, if you prefer, to actually help you with other people's titration schedule requests to come, as my way of giving back, to help others who are helping me now, as well as in the future.

Hi Christiana,

I am not going to draw up theoretical schedules - I just don't have the time. If you switch to titration at some stage, I will draw up a schedule based upon the figures you provide. I cannot determin if your sample titration schedules are correct, not least because the cuts carry no dates. Even if they did, I would not feel comfortable stating as to their accuracy. I employ my own spreadsheet to generate my schedules - I feel confident about my own schedules. To review your schedules would be too time consuming. Sorry.

As for your present situation: if your dose is 35mg Valium, and unless you know from past experience that you cannot taper off your Valium without employing titration, then it would make more sense to taper off by pill-splitting your tablets. I am not aware of anyone needing to titrate from a dose of 35mg Valium (a few people titrate when on a low dose of Valium), and particularly if your doctor is willing to prescribe 2mg Valium tablets.

At your present dose of 35mg Valium per day, 2mg tablets are not usually necessary either. If you were to reduce your dose by a quarter of 10mg tablet (2.5mg), this would equate to a drop of about 7% of your dose. The vast majority of people find this an acceptable cut when taking Valium. However, it would be sensible to obtain more 2mg (or perhaps 5mg) tablets now, so that you have them available when you feel the need to start making smaller cuts (generally, people feel the need to reduce the size of their cuts as their dose decreases).

Colin,

If you'd just taken a little time to read my posts, then you wouldn't have wasted your time writing half or more of what you've just written to me.  Everytime I ask someone about getting a titration schedule, I'm always referred to you to get one, yet I never do receive one, even after I've given you all of the information, to the best of my ability, you say you need.  If I don't have a schedule in hand, the next time I go to see my doctor, even if it has to be changed, which, as you said, most likely will be the case, as it's a common occurrence, for most people, my doctor may very well become very irritated with me and not be nearly as supportive as he is now, as far as me becoming benzo free, which could potentially harm me, in the long run.  Rightfully so, that really concerns me and is only feeding into my fear of becoming benzo free, which I hope you can understand.

Like I said, I feel like I'm on some sort of merry go round with you, as far as getting a schedule from you, as I've already asked you for one more than once and provided the information you say you need to make one more than once also, yet I've yet to get one from you.  On top of that, I'm physically and mentally exhausted, from being so sick already, and now because of what's happened between us, I'm very frustrated, with all of the back and forth of information, which has already been exchanged between us, yet I'm still left empty handed without a schedule.

If you don't want to find the time to help me, then just say so and I guess I'll either, hopefully, find someone else that will or just become benzo free the hard way.  I honestly don't know what more I can do or say anymore to get you to provide me with a schedule, except to tell you to take some time to read my posts, so you have a really good understanding of my situation and then you should, at least, be able to provide me with some sort of schedule to, at the very least, appease my doctor, at the moment, which should also serve as some sort of starting point for me, as far as becoming benzo free the easiest way possible.  Obviously, I'm unable to make a schedule on my own, else I would, since I've continued to ask you for your help with one and provide it to me.

I'm so upset now over everything that's transpired between us, my already very delicate central nervous system has started causing me even more problems, at the moment, and I'm left to suffer alone, as well as in fear of what might lie ahead of me, if i don't become benzo free the right way, with someones help with providing me a schedule, which is certainly unfair, unfortunate, and shouldn't have ever had nor have to happen, in the first place.  If explaining to you how the whole situation, which has transpired between us, has made and is currently making me feel, causes you to desert me or even worse, ban me from your website, then I guess just do what you have to do.  I just couldn't nor can't continue to carry this burden between us, about me getting a schedule from you anymore and am now having an even more hard time dealing with it than ever, right at this very moment in time.

I think I'll go have a good long cry over all of this now and, hopefully, I'll feel better and come back.  If I choose to never return here again or am no longer able to return here, not of my own doing, but all because of this totally frustrating circumstance I'm currently experiencing and perhaps the fact I've took a stand and voiced my frustrations, then I wish all my benzo buddies nothing but the very best with their journey to become benzo free and thank each and every one of you, from the very bottom of my heart, who've reached out to me and given me such well needed support, during what I consider one of the greatest trials of my life.  I simply just can't take anymore of this.

Christiana,

I am a volunteer (as is all of the team), and as I own and this website, so I have 1001 things to do.

I merely tried to save you a whole lot of trouble if you followed what is probably a completely unnecessary liquid titration schedule.

My last post included practical instructions of how you might proceed by pill-splitting, and what a quarter-pill reduction (2.5mg) would equate to in percentage terms. You would make these cuts as and when you feel able. The majority of our members probably would make these size cuts every 1-2 weeks in your position, but your milage might vary.

I have no idea why you think you might be banned from here? We only ban members for serious or continual rule-breaking. Clearly, you do not fall into either of these categories.

I've drawn up a liquid titration schedule for you. Although, I must say, unless you have already failed to reduce your Valium dose via pill-splitting, liquid titration is overkill. I suggest that you print off my other suggestions above, and not just this titration schedule, as this schedule is very long - perhaps far too drawn out for your doctor to approve.

If you use the titration schedule, please place the following code in your signature line (through your profile). You should print off a copy too!
